If you’re navigating the complexities of obtaining a pre-existing condition form for private health care in South West London, you’re certainly not alone. My partner is looking to have her mole examined privately, but given that we’ve only been with Vitality for under a year, we need to obtain a form from her GP that outlines her medical history. Unfortunately, her current GP in Norwood has quoted a staggering £150 just for this paperwork.
